The gap is where you stand

A gatekeeper is anyone close enough to notice that something is wrong, and willing to ask.

Recommended by the World Health Organization and named in the Government of India's National Suicide Prevention Strategy.

QPR Institute India is the India entity of the QPR Institute, which has trained gatekeeper instructors in more than a dozen countries since the 1990s. The method is evidence-based and peer-reviewed.

What a gatekeeper actually does

In CPR, the public is taught to recognise a heart attack and act before the ambulance arrives. QPR applies the same idea to a suicide crisis. It is not counselling, and it is not treatment. It is the part that happens first.

  1. Question

    Ask directly whether the person is thinking about suicide. Asking does not plant the idea. It is usually the first time anyone has given them permission to say it out loud.

  2. Persuade

    Listen without arguing, and stay with them long enough to agree that they will accept help. This is a conversation, not a negotiation.

  3. Refer

    Connect them to someone qualified — a counsellor, a doctor, or Tele MANAS on 14416. Where you can, make the call together rather than handing over a number.

Where this has been done at scale

With the Government of Andhra Pradesh's Department of Medical Education, under Project EASE, we trained first-year medical students in peer mental health support and QPR gatekeeper skills between May and September 2025.
